PILAR, Bataan – The Philippine Statistics Authority (PSA) Bataan Provincial Statistical Office opens Batch Requests Entry System (BREQS) outlet for PSA copy of Certificate of Live
Birth, Marriage Certificate, Death Certificate and Certificate of No Marriage (CENOMAR) at its office, Bataan Government Center, Alauli, this town Bataan this October 2017.
Under the BREQS system, the processing time for civil registry documents is between seven to ten days. PSA has already accepted and released requests from clients since July 2017 during the soft opening and trial period.
Clients may avail of the civil registry services from Monday to Friday, 8:00am to 5:00pm (no noon break) following these steps:
Application of PSA Civil Registry Documents
1. Secure Application Form and fill up the required information from the AF. Submit it to the receiving window.
2. Pay at the payment window.
3. Receive the claim stub with date and time of release.
Releasing of Requested Documents
1. Return to BREQS outlet at the date of release or upon notice of availability, if earlier. Present a) Local Official Receipt, b) ID, c) claim stub
2. Receive the civil registry documents after signing at the release portion of the application form, releasing log and duplicate claim stub.
The rates of fees of civil registry documents are P180.00 for Certificates of Birth, Marriage, Death and P235.00 for CENOMAR.
The Philippine Statistics Authority is the major statistical agency responsible in collecting, compiling, classifying, producing, publishing, and disseminating general-purpose statistics as provided for in Republic Act 10625. PSA’s function includes preparation and conduct of statistical survey on all aspects of socio economic life including agriculture, industry, trade, finance, prices and marketing information, income and expenditure, education, health, culture and social situations as well as the government and the political sector for the use of the government and the public.
